Consultation |

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. |
Please assist us by providing the following information at the time of your consultation: |
Your surgical referral slip and any x-rays if applicable.
A list of medications you are presently taking. |

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit. |

A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. Please have nothing to eat or drink after midnight prior to your surgery. You will also need an adult to drive you home. |
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.). |

X-Rays |
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility. |

CT Scans |
Computed Tomography (CT) scans may be required and your consulting/treating surgeon |

treatment rendered in order to avoid costly misunderstandings. |

Estimates: |
We offer individual estimates for each surgical treatment recommended by the doctor. At your request, we can submit a pretreatment estimate to your insurance to ensure accuracy of covered benefits. It cannot be assumed all professionals services will be covered by your insurance company. The financial obligations for treatment rendered are ultimately your responsibility. Estimates are designed to give each patient a general description of their benefits and is never a guarantee of payment from your insurance company. Of course other expenses may arise depending on healing and/or changes made in the treatment if necessary. All fees provided are valid for 90 days. |

Insurance: |
We are providers for Many insurances. Some health insurance group polices pay fixed allowances for certain procedures while others pay by percentage. Some dental carriers require the medical policy to be billed first for certain oral surgery procedures. In this case should the medical choose to accept the charges,
this could result in a large out of pocket deductable and your estimated portion may be more than we quoted. This also could result in your dental carrier to deny the claim even if the service is part of your dental plan. Please review your benefit book for a complete explanation of coverage. Should you have any questions regarding your specific plan, our office would be happy to assist you in reviewing your coverage if provided with your insurance information. |

Payment: |
Patient for services are due at the start of each surgical procedure. We accept Visa, MasterCard, check or cash. We offer Chase Credit and CareCredit, a healthcare credit card specifically designed to pay for treatments and procedures not covered by insurance. Apply online
